TeleForm Metadata Extraction

Description

Upload your TeleForm form template and generate:

  1. A Data Dictionary containing metadata on each variable in your form
  2. A Stata do-file that will label variables and values in datasets created by scanning your forms
  3. A SQL file containing a SQL Server CREATE TABLE statement

NOTE: For variable and value label information to be extracted you need to have entered this data using TeleForm Designer.

  1. Variable label is extracted from the variable's Description box
  2. Value labels are extracted from the Display section of a Choice field's properties

Get Your Form Template

  1. Open your form in the TeleForm Designer
  2. Right-click somewhere in the body of the form and select "Export to File"
  3. Choose "File Exchange Format (FXF)" and select your study's TeleForm directory as the destination (for example, \\m-ictfs1\TeleForm\<study>)
  4. Click "Export":
    • Uncheck "Include ID and ref marks"
    • Select "No graphics"
  5. Use Windows Explorer/My Computer to navigate to the file you exported. It will be named with the the form ID number e.g. 12345.FXF
  6. Right-click the file and on the 7-Zip menu select "Extract Here"
  7. Find the extracted file (called "Form.XML") and rename it using the name of the PDF version of your form

Get Your Metadata Files

  1. On this page, click "Browse" and select the form template XML file
  2. Click "Extract Metadata"
  3. The file will be processed and the metdata extracted
  4. You will now be able to download three files:
    • Data Dictionary: select the format you want and the delimiter (comma, tab or pipe)"
    • Stata Do file
    • SQL Server CREATE TABLE statement
  5. Click the "Download" buttons to open or save the files
  6. Save the files you download using the name of the PDF version of your form followed by the appropriate file extension: ".do" for Stata do-files, ".csv" or ".txt" for data dictionaries
  7. To return to the file upload section click "Clear"
